Assists licensed personnel by performing routine functions of basic nursing care to residents. Performs all nursing duties in accordance to the current Federal, State, and local standards, guidelines, and regulations governing the facility; provides physical support by transporting residents, assists or feeds residents, keeps incontinent residents clean and dry, gives, baths, tubs, and showers, assists residents to dress and undress, serves nourishments & supplements, answers call lights, ensures the safety of resident by following established procedures, other duties as required. Successful applicants will perform activities of daily living, feed and transport, residents in our LTC facility. Current NYS Certification required. Non-NYS certified Nurse Aides must apply for Reciprocity. GPN Presently accepting applications for Graduate Practical Nurses prior to taking LPN boards. Candidates will perform patient care duties and patient treatments until receive LPN. Will train. Excellent benefits available. Shifts available: 7-3 or 3-11. Must be able to train at least 8 weeks on 7a-3p shift. LPN Staff Nurse FT/PT 3-11 or 11-7 (w/ LTC exp) shifts. Will be responsible for daily assigned duties by the Head Nurse or Charge Nurse, which many include: Administration of medications (35-bed units), treatments, direct patient care, drug ordering as needed, maintaining records and forms in compliance with facility's policies and government regulations, and other duties as assigned for a 35-bed unit. Previous Charge and Medication experience in Long-term care required. Nurse Aide Training Nurse Aide openings are currently available at James Square. Successful candidates will be trained in a 6-week unpaid training course to become NYS certified. Must be available to work Full-time on the 7am-3pm or 3pm-11pm shift as a Nurse Aide after the completion of the training program. Previous session attendance will be verified. RN Clinical Informatics Specialist JAMES SQUARE has an excellent opportunity available for an experienced RN to teach and develop computer databases. Position is part-time 20 hrs per week M-F. BS degree and 30 hrs of continuing education in Informatics required with (practiced min of 2,000 hrs in Informatics Nursing within the last 3 yrs, or have practiced a min of 1,000 hrs in Informatics Nursing in the last 3yrs and completed a min of 12 cred hrs Informatics academics, or have completed a graduate program in nsg Informatics containing a min of 200 hrs of facility supervised practicum in informatics) with LTC or Med-Surg experience. Position will be responsible for the planning, development, organizational implementation and ongoing evaluation of automation in clinical and nursing practices by facilitating the use of technology within the clinical area. This position serves as the key clinical liaison to the development and production for all electronic systems impacting clinical processes and provides leadership for planning, design development, and management of clinical innovation through information systems, technology, education and clinical practices. RN MDS Program Coordinator Position includes managing the MDS team, scheduling care plan meetings, data input, interpreting DOH memos and training staff. Additionally will complete care plans and MDS forms on residents, Develops and implements the comprehensive care plan in collaboration with other disciplines. Reviews the care plan prior to being placed in the medical record. Attends and participates in comprehensive care plan and discharge meetings for assigned residents. Completes all 5, 14, 30, 60 day MDS booklets on Medicare Part A covered residents. Follows all covered residents for changes in condition and need for significant change books. Works in conjunction with the Nurse Manager to assure required documentation is completed by staff and is in compliance with James Square policies. May act as Charge Nurse as needed. Candidate must be a Registered Professional Nurse currently licensed in NYS; minimium of one year Long Term Care and MDS experience experience desired. Position responsible for the overall functioning of nursing service on respective unit on a 24-hour basis. Job Duties include, but are not limited to: completion of the MDS and care plan based on the nursing process, documentation, staff performance, staff meeting and leadership of the unit; organizes the unit and provides leadership for all staff in a assuring care plan compliance with James Square policies and government regulations; develops and implements the comprehensive care plan in collaboration with other disciplines; completes MDS booklets for all covered residents & follows patient changes; assures required documentation is completed by all staff and is in compliance with James Square policies; conducts unit meetings for all shifts to meet the needs of the unit; facilitates the development of staff by participation in orientation and performance appraisals; responsible for identifying problems and taking corrective action; counsels and disciplines staff as needed; administers medication and treatments as needed; reports and investigates residents complaints. $2,500 sign-on bonus. Opportunity for overtime pay as desired. RN Staff Development Educator Full-Time. M-F, 8a-4p. Do you like being a Preceptor and training staff? Would you like to work with new employees teaching them our policies and procedures? Primary job duties will include new staff orientation on nursing units, clinical training, and computer software training for staff. Must be a Registered Nurse with at least 1 yr. long term care experience. CPR certification instructor, staff education, and acute care experience preferred. Primarily a Monday - Friday, 8am-4pm position. Position will assist Staff Development Coordinator in all aspects of employee orientation, required documentation, and employee retraining. Position will require occasional work during the 3-11,11-7 or weekend shifts for training. RN Staff Nurse FT/PT 3p-11p or 11p-7a shifts. Charge and/or pass meds on a 35-bed unit. Position will be responsible for the following: Supervision of nursing care; promoting observance of nursing service policy and procedures; making appropriate rounds to assess patient/residents needs and nursing service being administered; assess change in patient condition and report to physician when appropriate; supervises development, use and update of patient care plan, reviews and investigates incident reports; evaluates personnel performance and attendance, and make staff recommendations; issues verbal and written warnings to staff documenting specific areas for improvement; assists Director of Nursing with improving nursing services; other duties as assigned.
